1. COMPLIANCE WITH RULES AND CONDUCT OF GUEST - Guest agrees to be subject to the control and guidance of WestSide staff while
on the premises and will follow instructions of WestSide personnel. Guest agrees to conduct himself or herself in a quiet, well mannered fashion while
on the premises and reserve all criticism of any major kind about other club Guests, guests or club personnel until a private meeting with a WestSide
manager in a club office can be arranged. Guest agrees to abide by all rules and conditions of Guest privileges contained in this contract or in the future as
prescribed by WestSide, and WestSide reserves the right to revoke or terminate the Guest privileges if Guest fails to keep and obey any such rules and
conditions and failure to follow said rules does not void Guest’s financial obligation. Guest further agrees to abide by all additional WestSide
guidelines. Any violation or failure to abide by rules, guidelines and policies will result in immediate termination of guest privileges.

2. USE OF FACILITIES BY GUEST - Guest agrees and represents that all exercise, treatments and use of all club facilities shall be undertaken at
Guest’s own risk. Guest does hereby expressly forever release and discharge WestSide and any beneficial owner of WestSide and any affiliated
companies and their respective agents and employees (the “WestSide Parties”), from all such claims, demands, injuries, damages, actions, or causes of
action. In the case of any accident, Guest agrees that she/he will be examined at his/her sole expense by a licensed physician who shall report in
writing to both Guest and WestSide.

3. PERSONAL PROPERTY – The WestSide Parties shall not be responsible for damaged, lost or stolen articles of clothing, damage to automobile, or any
other personal property of any Guest or staff. Guest understands the lockers in the locker rooms or other areas of WestSide are to be used only while
Guest is using the facilities that day and that it is Guest’s responsibility to bring a lock to secure their day locker. If a lock is left on a locker
